Privacy policy

How your personal and health information is collected, used and protected.

About this policy

This policy explains how Dr Travers Weaver’s practice (“we”, “us”) collects, uses, stores and discloses personal and health information. We are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and, for patients seen in New South Wales, the Health Records and Information Privacy Act 2002 (NSW).

What we collect

  • Your name, date of birth, address, phone number and email
  • Medicare, health fund, pension and concession details
  • Health information, including your medical and eye history, medicines, test results, images and referral letters
  • Details of your referring doctor or optometrist and other treating practitioners

How we collect it

We collect information directly from you, from your referrer and other treating practitioners, from hospitals, and through the enquiry forms on this website. You may use a pseudonym for a general enquiry, but we need your identity to provide medical care and to claim Medicare or health fund benefits.

Why we use it

  • To provide assessment, treatment and follow-up
  • To communicate with your referrer, GP and other treating practitioners
  • To arrange tests, surgery and hospital admission
  • For billing, Medicare and health fund claims
  • To meet legal, accreditation and professional obligations

We do not use your information for marketing, and we do not sell it.

Who we share it with

We share information only as needed for your care or as the law requires or allows. This may include your referrer and GP, other specialists, hospitals and day surgeries, pathology and imaging providers, Medicare and your health fund, and our IT and practice-software providers. We do not usually send personal information overseas; if a service provider stores data outside Australia, we take reasonable steps to make sure it is protected.

Website enquiry forms

When you send an enquiry through this website, the details you enter are sent to the practice through our website host (Netlify, Inc.), which may store form submissions on servers outside Australia. Please keep detailed medical information out of website forms; we will collect it when we speak with you. This website does not use advertising or tracking cookies. Fonts are loaded from Google Fonts, which may record your IP address.

Storage and security

Records are held in secure practice-management systems with restricted access, and paper records are stored securely. Medical records are kept for the periods required by law, then destroyed securely.

Access and correction

You may ask to see or have a copy of your information, or to correct it, by contacting the practice. We will respond within a reasonable time, usually 30 days. A fee may apply for copies, and in limited cases the law allows access to be refused; if so, we will explain why.

Complaints

If you have a concern about your privacy, please contact the practice first so we can try to resolve it. If you are not satisfied, you can contact the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992), or for NSW health records, the NSW Information and Privacy Commission (ipc.nsw.gov.au, 1800 472 679).
